Direct Billing Pilot
We are trialing a limited direct billing process for eligible Blue Cross Blue Shield (BCBS) members, including Federal Employee Program (FEP) and Global Solutions members. Direct billing is not available for U.S. domestic members.
Cedar Medical Clinic Tokyo remains an out-of network provider. Direct billing is available only when the insurer provides a written Guarantee of Payment (GOP) and our clinic confirms acceptance before your appointment.

BCBS has advised allowing
approximately 5–7 business days to arrange a GOP.
Do not assume direct billing is approved until we confirm it in writing.
How to Request Direct Billing
There are 4 steps.
Your insurance card alone does not establish eligibility or guarantee payment.

Contact us in advance
E-mail us at least 7 business days before the visit. Include your name, preferred date, reason for the visit, and a picture of the front & back of your insurance card.

Insurer authorization request sent by Cedar Medical Clinic Tokyo
We will send the anticipated services needed to your insurer and help coordinate the GOP request. You may need to contact your insurer, confirm overseas benefits, and complete or sign plan-specific authorization or claim forms.

Receive written confirmation
If a GOP is issued, we will review the authorized services and amounts, then confirm whether your visit can be handled through direct billing.

Attend your visit
We bill the insurer for approved services.
You pay any applicable copay, deductible, coinsurance, or costs outside the authorized coverage.
Eligibility & Restrictions

Case-by-case approval: not all BCBS plans have applicable overseas benefits or qualify for direct billing.

Out-of-network: Cedar is not a contracted/in-network BCBS clinic.

GOP required: the clinic must receive and accept a written guarantee before the appointment.

Service limits: only services and amounts specified in the GOP are eligible for this billing arrangement.

Patient responsibility: deductibles, copays, coinsurance, noncovered services and amounts not authorized by the insurer remain the patient's responsibility.
Need care sooner?
If a GOP cannot be confirmed before your visit, you can still book an appointment as a self-pay patient. Payment is due at the time of care. We provide an English itemized receipt and supporting documentation so you can submit a reimbursement claim to your insurer. Please do not delay urgent medical care while waiting for insurance authorization
